What should a new patient bring to First Choice Medical Center?
If you are new to us, please bring a photo ID, your insurance card, a current medication list, any recent records you have, and the main questions or concerns you want to cover. If you have home readings, specialist notes, hospital records, imaging reports, or recent labs, bring those too.
Should new patients complete forms before the visit?
Yes, when you can. Completing your forms before the appointment lets us have your basic information ready. If a form is unclear, unavailable, or asks for something you're not comfortable submitting online, please call us before sending private medical details.
Should new patients bring outside medical records?
Yes. Outside records help us understand your prior care and what you may need next. Helpful records include recent labs, imaging reports, specialist notes, hospital paperwork, medication history, and prior diagnosis information. If you can't get the records themselves, the names of clinicians or facilities still help.
How should new patients list medications?
List each medication with its name, dose, how often you take it, your pharmacy, and the reason if you know it. Include over-the-counter medicines, supplements, allergies, and any recent changes. If writing it all out is tricky, just bring your bottles, photos, or a pharmacy printout instead.
