When should I use First Choice Medical Center patient forms?
Use our patient forms when we ask you for new-patient paperwork, records release documents, or other materials to prepare for your visit. Forms help us gather information ahead of time, but they don't replace a visit, clinician review, or a direct call when your question is urgent.
Should urgent medical questions go through forms?
No. Forms are not the place for urgent medical questions. For emergency symptoms, call 911. For urgent office questions, please call us directly. Online forms may not be reviewed right away, so don't use them when symptoms or time-sensitive medication needs require prompt attention.
What if I cannot complete a patient form?
Just give us a call before your appointment if a form is unavailable, unclear, or hard to complete. We can explain the next step and let you know whether you need the form before your visit. Please don't send sensitive medical information through an unsure channel.
