Please bring your photo ID, current insurance card, medication list, allergies, pharmacy details, any recent records you have, and the main questions on your mind. If you're new, take a look at new-patient preparation and patient forms before you come in.
Call us when it's urgent
Web pages, forms, email, and portal messages may not be reviewed right away. For emergency symptoms, call 911. For urgent office questions, call 928-888-9750 rather than waiting on an online reply.
