This applies to the clinic's health programs and activities.

Free help for people with disabilities

The clinic provides reasonable aids and services at no cost to help people with disabilities communicate effectively, such as qualified interpreters and written information in other formats when available and appropriate. Contact the clinic to request this help.

Free language assistance

The clinic provides language assistance services at no cost to people whose primary language is not English, such as qualified interpreters and information written in other languages when available. If you need these services, contact First Choice Medical Center at 928-888-9750.

Free language assistance is also available in other languages. Please ask the clinic and staff will arrange support.

How to request help or raise a concern

To ask for an interpreter, an accommodation, or information in another format, or if you believe the clinic failed to provide these services or discriminated in another way, contact the clinic at 928-888-9750 or Info@1stchoicemed.center. Do not include protected health information or private medical details in non-secure messages.

File a civil rights complaint
