Free help for people with disabilities

How to request help or raise a concern
To ask for an interpreter, an accommodation, or information in another format, or if you believe the clinic failed to provide these services or discriminated in another way, contact the clinic at 928-888-9750 or Info@1stchoicemed.center. Do not include protected health information or private medical details in non-secure messages.
File a civil rights complaint
